Wooden Nickel Program
We have started a new and exciting program effective May 1, 2003 for all active patients with appliances and braces. The purpose of the program is to reward and encourage patient cooperation, which will allow orthodontic treatment to progress on schedule and finish with excellent results.
Program:
At each regularly scheduled appointment, a patient may qualify for up to (3) three wooden nickels. The wooden nickels may be redeemed for various prizes. The prizes offered may change from time to time due to availability without notice. A current prize list will be maintained in the reception area indicating the number of points needed. Each wooden nickel is worth 10 points.
One wooden nickel will be awarded for each of the following areas:
- Arrive ON TIME for scheduled appointment time, not missing the previous appointment. You must ask the receptionist for this nickel when you arrive.
- Maintain excellent oral hygiene. All tooth surfaces and appliances are clean; no food or plaque is trapped in or around the braces, and little or no swollen or red gums. The clinical assistant will give this nickel to you.
- Good patient compliance. No broken or loose brackets or bands, no bent wires, wore elastics, headgear, or appliance as instructed. The clinical assistant will also give this nickel to you.
Please Note:
Wooden nickels have no cash value. The patient will be given a maximum of 60 days after appliances or braces are removed to redeem for prizes. The patient incentive program may be discontinued after 90 days noticed by a sign placed in the reception room of the office. All wooden nickels are the property of Easton Orthodontic Associates. Unfortunately lost nickels cannot be replaced. |
